Job Description

Provide Casework and Counselling to teenage girls in a Residential setting
Liaise, collaborate with, and provide information to external partners and stakeholders
Coordinate and manage all external appointments of the residents
Assess risks and needs of residents and their families and formulate the Individual Care Plan (ICP) for each resident accordingly.
Ensure execution of ICP’s planned action according to goals, in close collaboration with care staff and programs team
Provide counselling to resident’s family, and conduct restorative work between residents and their families
Monitor daily incidents and progress of residents
Provide inputs at weekly Disciplinary Committee conferences and ensure execution of outcomes derived from the conferences
Ensure and maintain accurate and updated records on each resident
Prepare, review and submit all regular and ad-hoc reports required
Conduct group work with residents and/or with their families to equip them with the necessary life skills
Review relevance of all external proposed classes/workshops/talks that may be useful for residents
Coordinate with the respective agencies on the proposed sessions, internal team and Programs department to facilitate sessions
