Program Coordinator /Patient Service Associate (Home Care Team, 2 shifts) #HKN

Location: West

Working hours: Any 5 days per week, including both weekends & PH.

2 Rotating shifts (by roster): 8am – 6pm; 12.30pm – 8.30pm

Salary: up to $3000, $18/hr for locum roles

Job Responsibilities

• Support planning, assessment, and ongoing monitoring of programme operations to ensure high-quality patient care.
• Coordinate with external vendors to arrange patient transport and courier services.
• Liaise with hospital bed management units to facilitate bed requests and allocation.
• Coordinate with multiple departments to schedule appointments and ancillary services as requested by clinicians.
• Manage patient admissions, discharges, and transfers within the Electronic Medical Record (EMR) system.
• Provide financial counselling to patients and next-of-kin on estimated bills, available subsidies, payment schemes, and modes of payment.
• Assist with completion of payment documentation and ensure proper follow-up for cases through to discharge.
• Generate and export reports from EMR systems and update relevant databases to support clinical operations.
• Maintain accurate documentation and ensure timely information flow across teams.

Job Requirements

• Min Diploma in any discipline.
• Proficient in Microsoft Office applications and computer systems.
• Willing to work beyond regular hours, including weekends and public holidays when required.
• Able to thrive in a fast-paced environment and manage multiple tasks effectively.
• Strong team player with a positive and proactive work attitude.
• Prior experience in a hospital or healthcare setting is an advantage.
• 6 months Locum positions are available.
