Locum Medical Social Worker, Community Care (WEST) #HFT

Clinical Duties

1. Casework/Psychosocial Care

a) Provide end-to-end case management support and appropriate level of care through assessment, care planning, interventions and evaluation of care, including complex, specialized and high risk cases

b) Staff is able to provide professional advice or review complex cases

c) Advocate for the wellbeing of patients and beneficiaries

2. Group Work

a) Able to plan, lead and facilitate group work sessions independently and build social support systems for target groups

b) Reach out to individuals and/or target groups who may benefit from group work

c) Able to support the lead in the planning of group work evaluation

3. Community Work

a) Assist in planning and implementation of existing and/or new programmes which enhanced psychosocial well-being of the community

b) Participate in programme and activities evaluation

c) Deliver public education talks with guidance

d) Network with targeted community partner

4. Programme/Project Development and Implementation

a) Assist in the development and implementation of programmes

b) Lead department programme/project to improve the quality of care and services of patients and caregivers

c) Participate in inter-department programme/project to improve the quality of care and services of patients and caregivers

Education/ Clinical Supervision

a) Participate in developmental programmes, learning opportunities and supervision for personal and professional growth

b) Provide guidance and training to interns support staff and juniors

c) Provide clinical supervision to fieldwork interns and support staff

d) Assist to organise, coordinate and provide continuing education to department and inter-department colleagues and community partners

e) Assist to develop resource materials

f) Assist to conduct talks, professional workshops and seminars

Requirements:

• Degree in Social Work / Diploma in Social Work with strong experience
