Clinic Staff Nurse – Chronic Disease Management Diabetes Care #HYT

Job Scope:

• Provide nursing care and support to patients with diabetes and other chronic conditions
• Perform blood glucose monitoring and basic patient assessments
• Administer insulin and medications as prescribed
• Provide patient education on diabetes management, diet, lifestyle modification and medication compliance
• Assist doctors during consultations and clinic procedures
• Coordinate follow-up appointments, laboratory tests and referrals
• Monitor patients’ treatment progress and provide counselling when required
• Maintain accurate documentation in EMR
• Ensure compliance with infection control and clinic protocols

Requirements:
• Registered Nurse with valid SNB
